SMS Terms and Conditions

These SMS Terms and Conditions (“SMS Terms”) govern your participation in the Form Theory Skin Lab text message program. By opting in, you agree to these terms.

1. Program Description

Form Theory Skin Lab operates a recurring SMS marketing and informational text message program. Messages may include product announcements, new batch releases, health and wellness tips, exclusive offers, limited-batch alerts, and brand updates.

2. How to Opt In

You may opt in to receive text messages from Form Theory Skin Lab by:

Checking the SMS opt-in box on our website sign-up or checkout forms
Texting a keyword to our designated number as advertised
Providing your phone number and consent through any other Form Theory Skin Lab opt-in method

By opting in, you certify that you are the account holder or have the account holder’s permission to enroll the mobile number provided, and that you are at least 13 years of age.

3. Message Frequency

Message frequency is approximately 2–3 messages per day. Message frequency may vary based on promotions, product releases, or other brand activity.

4. Message and Data Rates

Message and data rates may apply. Charges are billed by and payable to your mobile service provider. Form Theory Skin Lab is not responsible for any charges incurred.

5. How to Opt Out

You may opt out of the SMS program at any time by replying STOP to any text message you receive from us. After opting out, you will receive one final confirmation message confirming your removal. No further messages will be sent unless you opt back in.

6. How to Get Help

For help, reply HELP to any message or contact us at formtheoryskinlab.com. You may also contact your mobile carrier for questions about message and data rates.

7. Supported Carriers

Supported carriers include AT&T, Verizon, T-Mobile, Sprint, Boost Mobile, Virgin Mobile, U.S. Cellular, and other major US carriers. Carrier support may vary. Form Theory Skin Lab is not liable for delayed or undelivered messages.

8. Non-Sharing of Mobile Numbers

We do not sell, rent, share, or disclose your mobile phone number to any third party for their marketing or promotional purposes. Your number is used solely to deliver messages in this program and is shared only with our SMS service provider (Twilio) for message delivery purposes under strict confidentiality obligations.

9. Changes to These Terms

Form Theory Skin Lab reserves the right to modify or terminate the SMS program or these terms at any time. We will notify you of material changes via text message or by updating this page.

10. Privacy Policy

Your use of our SMS program is also governed by our Privacy Policy, which is incorporated into these terms by reference.

Life-Changing Relief for Chronic Pain

I’ve dealt with ongoing pain most of my life due to a physical disability, along with lingering pain and inflammation from a car accident and broken arm over 12 years ago. I’ve also had foot surgeries on both feet, with plates in place. I started using this in February, and I honestly wish I had found it sooner. Omg this stuff literally saved my back mainly but pretty much my entire body. Let me tell you after a hard day at work having some of this rubbed into whatever spot is giving you problems. I’ve tried multiple different pain relieving gels, creams. ETC!! even over the counter medicine doesnt compare to the XTREME CBD Oil. I cant explain the pure bliss and relief this oil does for me. Its so healing, and makes everything so much better. My motto is if it hurts, slap some CBD on it.

Love it

I do love this stuff, however, I do have to reapply it in the afternoon as my face is very dry. If I wasn’t doing a lot of work outside, then I might not have to reapply. Other than that it is light upon applying, and my skin loves it, doesn’t leave it feeling oily. I don’t get any break outs from it, which is good for me, cause I usually do from other moisturizer’s. I believe I’ve found my moisterizer for life.

I have tried many CBD products, back history. I’ve had several back surgeries, and also need surgery on my neck due to ruptured discs. I’m done with surgeries! With that said, I ordered the lotion, and it is working very well, it helps with the inflammation and pain, better than other things I’ve tried, even the celebrity, and name brand ones. Trying the oil next!
